On signing this application form, I declare that -

• The particulars given in this form are true, accurate and complete to the best of my knowledge and belief.
• I have no pending or concluded criminal proceedings against me.
• I have never been declared as an undischarged insolvent.
• I hereby undertake to comply with the Bye-Laws, directions and guidelines issued or the resolutions passed in accordance with the Bye-Laws of the ICMAI SAO.
• I undertake that the information furnished by me is true & complete. If found false or misleading at any stage, my enrolment shall be summarily cancelled.
• I undertake to furnish any additional information as & when called for by ICMAI SAO.

• One Time Enrolment Fee: Rs. 2,000/- Plus GST @18% = Rs. 2,360

You are further requested to submit a Self attested copy of following documents along with Form A, to the registered office of ICMAI SAO at New Delhi address is mentioned below:

1. Form A to be filled by the SIA Entity and submitted along with other documents mentioned below.
2. Original one passport size Photograph of each partner/Director.
3. Self-attested Xerox copies of proof of address of SIA Entity.
4. Self-attested Xerox copies of CIN Number and TAN Number.
5. Self-attested Xerox copies of Identity Proof & address proof (Aadhaar Card & Pan Card of each partner/Directors).
6. Self-attested Xerox copies of educational qualification, professional qualification documents of partners /Directors.
7. Self-attested Xerox copies of NISM Certificate and SAO Membership Certificate OF SRO Registered partners.
8. Payment proof of one time Enrolment fee.
9. Financial statements / Income-tax Returns of SIA Entity for the last three years (Verified Acknowledgement).
